Mr X complained to Betsi Cadwaladr University Health Board (āthe Health Boardā) about the care and treatment provided to his wife. The Health Boardās investigation included a review of the oncology notes and speaking to staff involved in his wifeās care. However, it did not have access to his wifeās medical records prior to preparing its response. Mr X complained to the Ombudsman that he was unhappy with the Health Boardās response.
Following discussion with the Ombudsman, the Health Board confirmed that it had subsequently located the medical records. The Health Board agreed to provide a further written response to Mr X within 30 working days of the Ombudsmanās decision.
